Seeing your doctor about an anal problem

Almost everyone is embarrassed about seeing the doctor about an anal problem, such as anal pain, wind, or anal itching. It is important that you get over this worry, so that you get treatment for the problem. Value your own health and think of your doctor simply as someone who is helping to maintain your health. Bowel or anal symptoms, such as constipation or bleeding, can sometimes be serious. People are literally dying of embarrassment because they do not see their doctor when bowel symptoms start, often because of a fear of examination of their back passage (rectal examination). Therefore they may reach their doctor only when bowel cancer is quite advanced. Although bowel cancer is much less common than piles, it is always a possibility and can be treated very successfully at an early stage.

Take control

Remind yourself that you are in charge of the consultation with your doctor. He or she cannot examine you without your permission. Therefore it is perfectly OK for you to visit your doctor and say ‘I have such-and-such a problem. At the moment I just want to discuss it. I don’t want to be examined’. When you have got over the hurdle of talking about the problem you may later feel quite alright about being examined, but it is your decision.
 

Think the problem through

What do you think will be going on in your doctor’s mind while you are being examined? Are you imagining your doctor will be mentally rating and criticizing your appearance? In fact, in this situation your doctor is interested in your insides, not your outsides. By the time he or she becomes a GP, your doctor will have seen literally thousands of backsides and is not interested in their appearance. In fact, he or she will be thinking about the symptoms you have described, and whether they match up with the findings on examination.
 

Take a friend

Would it help to have a friend with you? You might feel this would make it worse, but the right person can be reassuring, perhaps someone who has had a lot of medical treatment themselves and so is matter-of-fact about being examined.
 

What is involved in the examination

If you are a female and the doctor is male, he will ask a female nurse to be present during the examination. Like any other part of the examination, examination of the back passage is methodical, and all doctors are taught the following procedure.
 
The patient is asked to lie on the left side with the knees drawn up to the chest; this position helps to relax the muscles around the back passage. If you are feeling tense, breathe slowly and deeply with your mouth open. The doctor puts on a pair of gloves and separates the buttocks slightly to look for abnormalities such as piles, cracks in the wall of the anus (anal fissure), skin tags or warts. The doctor then puts some lubricant on the gloved finger, and places the fingertip on the anus, pressing slightly. When the anal muscle relaxes, the finger is inserted slowly and the tip is rotated gently to feel for lumps and bumps and specific structures (such as the prostate in men).The finger is withdrawn slowly and then the doctor inspects the glove for blood or pus. It is not painful, so if you experience any pain, tell your doctor. You may just experience a feeling of rectal fullness during the examination, as if you wish to have your bowels open.
 
Your GP might suggest inserting a short hollow instrument (proctoscope) to separate the walls of the bowel so that they can be seen clearly as the instrument is withdrawn. Again, this is not painful and your GP will not do this without your permission and explaining it to you.

Hello, I have Recently have had a fairly amount of Blood coming from my Rectum, I am like most Very Nervous to go see a Dr. 1.) Because I'm Scared. 2.) Because i dont have medical Insurance. This whole Bleeding process has lasted about a Month, I kept telling myself it will Eventually go away, Well today it was an amount of Blood that Scared me and pushed me to get it fixed. Any Professional Suggestions?..

Hi, I have noticed fresh red blood in my anus, it happens whenever I poop or pee. I feel no pain, but I can see drops of fresh red blood. This has been ongoing for almost a month. I don't feel any difficulty or pain whatsoever if I poop or pee. Can you help me please? I am scared.

Please can anyone help? I have an extremely embarrassing problem which I can't talk about. I am sexually active and have a wonderful lover. But recently during lovemaking I have ejected very small amounts of faeces. I noticed the sheets were soiled to my utter horror!It's not much, but enough to be seen. Happily my partner has not seen it as I hastily cover up the evidence. But one day he will - and i dread it! Has anyone else experienced this? I feel completely mortified and ashamed.

I would firstly like to reassure everyone that the doctors trip is not bad at all. I am a male, and had a female doctor do the examination, but I felt comfortable all the time, and she explained everything as she went along. The problem is not solved however. I went in due to pains after passing a stool or exercise (straining the muscles i suppose.) And i was prescribed some local painkilling and hemmarhoids cream, which when i was using it stopped the pain. The blood was still present on the paper however. I am still experiencing pain and blood on the toilet paper at times, and it is stopping me from doing sports due to pain. Also, i feel my anus is a bit leaky. Is anyone else experiencing this problem, and seen a doctor that has solved the problem?

Sir, a Doctor three days diagonosed my pile without protoscope and found that it is in third stage.In this regard,I would like to ask whether there is role of digestive system(liver and associated glands) in maintaining proper digestion ultimately culminating into piles?I have the habit cleaning my anus 2.5"with my index finger then only I get satisfaction after defaecation.But I have read related articles in this and now I am highly alarmed.thanks

@PaulMD : I disagree entirely. I am a doctor in my 20s. I have performed many, many rectal examinations on many, many different people. I am heterosexual and have a great relationship (including sexual!) with my girlfriend. However, when examining a rectum, on ANYone, my mind couldn't be further from sex, perving at a body or anything similar. I gain no pleasure out of the procedure, just in the same way that I gain no 'pleasure' out of looking in someone's ears. In the same way, I also do not dread the procedure just because it's near someone's "naughty bits". It is an examination technique just like any other. The only thing to note is that, in general, a DRE is easier on a very thin person compared to a very obese person (same goes for examining a shoulder actually!) If doctors were unable to separate out work and 'play' then 'playtime' at home with our partners would become very wierd and very UNpleasurable'.

If the patient is a 20 year-old sprinter, it’s a near certainty that the thoughts running through this hypothetical male doctor’s head won’t be 100.00% professional. He has a penis. He has testicles. And unless he’s in his 60’s, he’s probably got a good amount of testosterone flowing through his body. And just because he’s the doctor and she’s the patient and he has to perform a rectal exam to diagnose a problem doesn’t mean that his interest in the situation is 100.00% medical.

Hello.. Well its quite embarassing to have a pain in ur anal region, but i think its very necessary to tackle it on time. My problem is a painful sensation in my anus anytime i tense the muscles there..i have never had anal sex..it just started a week back when i was stooling, and i have noticed no blood or pus. Please i need advice or better still referral to ab good doctor i can have a chat with. I'd like to use a doctor other than my family doctor. HELPPPPPP

Thank you. My mother & grandmother both died from Bowel cancer so you always have a nagging doubt about blood from behind. I now feel a bit more confident about a visit my G.P.

What if the bleeding of your anal is often? Sometimes it bleeds sometimes its not bleeding. Is it possible that there is a wound in the anal?

I have told my Dr. about this problem. I have it can be caused by stress. I am taking care of my fiance who has terminal cancer, Lost my mom to cancer and my second husband to cancer in 2003. I am, and was, a caregiver to them. Could this be caused by stress. I have also heard that policeman get this frequently from stress of their jobs. I will see my dr. again, and print this out for her to see. I got a lot of info from your site, and I thank you very much. It is happening right now and driving me to want to scream from the pain. Ive found out that i have one rather painful skin tag near my anus... (not on it) But ive looked up the symptoms when i first got it and i found hermorriod information. I FREAKED! I saw the part where hemorrids usually itch and mine didnt. i was so releaved. After about 2weeks with just putting germoline on it, i found that it started itching (not dramatically) but just slightly... i was so scared and booked an appointment with my local doctor. Im so scared so show him my rear!! :( i understand that he has probably seen lots of them, but that doesnt change my mind of being worried!! My appoinment is at 1:00pm today and im constantly looking at the clock.. :/

What kind of doctor do you see if you get a dull pain deep in the anus every 6 to 8 weeks. the pain wakes me up from a deep sleep where i will go to the toilet and sit trying to make a bm but i don't need to go. just he pressure of me trying to go will make the pain go away after about 10 min. thanks Jim

I've had anal itching for the past 6 months & I've been tested for every type of std. However, I haven't gotten tested for genital warts! It is very frustrating and haven't seek advice from a doctor yet. I do drink alcohol often and eat a lot of fast food. So I don't know if this is a cause. I need help! Anyone know what this can be??

My sleep is disturbed by the pain and irritation which causes me to make frequent visits to pass stool.Afterwards, the pain is severe for some few minutes and I sometimes notice bright red blood on the toilet paper.

uhm.. so this morning i took my morning bowel movements and when i wiped there was surprising a significant amount of blood on the paper... like when u have your period? (for those who can relate). i wiped again and there wasn't more blood, so maybe i just pushed a bit too hard? there's no pain at all. but im also expecting my period now-ish, so i dont want to confuse myself or get unecesarily paranoid. should i see a doctor? or just wait and see if the next poop holds the same experience? sucky much?!

had urge for bowel movement, started with small "rocks" in bowel movement and some nausea and pressure, then had diarrhea. Following this was continured pressure but when trying to relieve bowel only blood came from rectum. This continued off and on for about 7 hours

hello, im 13 and iv'e been experiencing pain's in my anal region when excreting. for a few weeks i have not been eating properly.(loss of appetite.)and because of this been relying mostly on junk food. i have had the pains for about 2 weeks, and bleed almost every time i go to the toilets, it wouldn't be much blood, but i'm very scared about it. my mum has piles, and iv'e heard that it can be hereditary.the pains can be agonisingly painful,causing me to tence. i'm sure the pains are due to my anus and not to constipation, as i have been on a VERY fibre high diet for the past few days(breakfast:fruit and fibre,lunch: assortment of fruit,dinner: rice like curry(s)and before going to bed:fruit and fibre again.)i forgot to mention that i am also a vegetarian,(if that matters... iv'e also been experiencing very irritating itching, usually at night... i have considered going to a doctor, but am VERY sensitive about him/her seeing my penis, though the article did mention the doctor concentrating more on my anus, rather than other places. i have broken my right wrist so it's in a cast, which means i dont get to wash as much as i'd like to (every other day or maybe more) wondering if this might have anything to do with the itching? well that's about it- i'd be grateful for any feed back that you could give me, about the pains, the itching,any ways i can improve my diet, and about seeing the doctor, thank you!

I've had a very tough time for the last 5 months or so. I began feeling like I have piles, then I saw some blood on the toilet tissue after wiping my anal hole. The itching became worse and I thought it's related to spicy food or "hot chillies". I went to my GP who referred me to a specialist surgeon. I underwent so many tests and got admitted for three days. After such many tests, the specialist said all my organs are fine, no infection at all. I was told I'm suffering from PROCTALGIA FUGAX, which is "just a severe pain" I was given painkillers (tablets) to take when there is pain attacking me. Right now the attacks from this pain are becoming regular, especially starting at 12 midnight last up to 4am. It is so painfull that I feel like dying when theis excrutiating anal pain is persisting. My GP has given me sachets of MOVICOL which he suggested that I must take ONE SACHET before going to bed at night. It does sometimes assist me coupling it with strong pain tablets. What is worse is that my erection has been badly affected presumably because of this persisting anal pain. I've also noticed that the pain attacks me severely after I've eaten hard bread or rolls. This condition is really taking its toll on me. Can someone advise me what else to do because medical experts are saying there is no specific thing I'm suffering from except this proctalgia fugax. I AM THE MOST WORRIED PERSON RIGHT NOW !!!!!

sir, i really don't know what to do because now when I wipe my anus with toilet paper sometimes i see blood because maybe i scratch it and it's bleeding, I so this treatment on this website to powder a cotton wool with baby powder and put it inside my bum area and it is helping Im wondering if I can just keep it that way until it stop's please reply cause I'm to embarrassed to even tell my parents

I am suffring from fissure and 2nd degre pile - sometimes it has got some releaf but again restart the problem, I have problem when I have in toilet, I saw with Doctor, he advised me for immediate operation but I feard because of longterm rest, I have no time. I have serious pain when i use the toilet, the pain goes up my back. sometimes just after using when standing up i feel a sharp pain for like 5 seconds and then the pain reduces. i have both a fissure and second degree pile..... it is so painful that i dread going to the toilet for number 2......

I am so much on pain!. It started with bleeding in the anus, I went to doctor ,gave me some cream and ok for a while. Later it became worst bleeding as I pass the bowels (not always) but the pain along up my rectus is horrible. I'm so scared. Two months ago I had a hysterectomy done, everything went and now I'm back with the pain inside me. What can I do?

I sometimes have anal itching and sometimes pain when i pass a stool.When I have bowel movements, I sometimes feel as though there is more to come but I cannot pass it. I have also had some bright red blood when I go to the toilet. Ther also appears to be a little skin tag/lump above my anal opening.I'm very worried about it and scared to got to a doctor.

hello i have had prostitis for over three years and i am on tamsulosin 400mg take one a day i get pain when i ejaculate and a hot burning sensation in my bottom they say there is not a lot they can do as they dont know a lot about the condition and i just have to learn to live with it is there anything that you tell me that would help many thanks gary

I suffered from an anal fissure a year ago, and it lasted almost 6 months. The first 3 months were the most painful, and I've never felt anything as bad as that (and I've given birth and broken bones before). So my heart goes out to anyone who's suffering from that horrible, awful, malady. I'll tell you exactly what helped me, as I did tons of research and tried everything under the sun. 1. Buy something called L-Glutamine pills and take 1 or 2 daily. You can find them at a local healthfood store because body builders take them as they help regenerate muscle tissue. Don't ask me why, but these helped more than any other pill I took. 2. Vitamins!! Take Vitamin C,E, and A once daily. C helps the immune system, E helps skin and muscle tissue, and I think A helps cel growth, I'm not totally sure, I just know they helped. (and really what harm can taking vitamins do anyway??) 3. Start taking a mild laxative every night before you go to bed. I experiemented w/ alot of them and I've found Miralax to be the best. Milk of Magnesia works well but it's soooo potent you've got to be very careful or you'll end up w/ a case of the runs the entire next day (no good, especially if you're at work/school!!) What I found the best was to take a cap full of Miralax w/ a full glass of water before bed. I would usually alternate between that and a 5 oz. cup of prune juice before bed followed by a cup of water. 4. If you're up for it, stop eating meat completely. Well I stopped eating red meat and chicken. I did eat some turkey in small portions. I ate tons of fruit and veggies, and beans. As gross as it may sound I swallowed a spoonful of straight olive oil every night before bed, because it provides a slick coating inside your intestines and makes things much easier going out, trust me. 5. This one is very important (well they all are really) and I still take tis stuff just cuz it's so good for you! Ground Flaxseed! You can also find this at a local healthfood store and it's not very expensive either. One bag is usually around 10-12 dollars and it lasts almost 6 months! You can sprinkle it on almost anything from salads, cereal, oatmeal, yogurt, soup, sandwiches, pizza, etc. I highly recommend this! Please buy the ground stuff, not the pill form as the ground stuff is way more effective. Not only does it have awesome Omega 3's, it helps you go to the bathroom very well!! 6. I got a perscription from a rectal surgeon for a cream called Diltiazem. You apply it to the outside of the affected area about 30 min. before a bowel movement and right after too. I know, I know..how do you know when you're going to go 30 min. prior?? Well after you start taking prune juice, laxatives, ground flaxseed, and eating lots of fruits/veggies you'll start to be on a regular schedule, trust me! After I started doing all of these things my pain went away greatly...oh, and I also took warm sitz baths about 2-3 times daily for about 15 min. each time. My pain really started subsiding about 4 months after I got the cursed thing, and I believe it went away completely about 6 months after I got it. I'm so deathly afraid of getting it again, I haven't stopped taking any of the above mentioned things to this day. Besides, all that stuff is really good for you anyways. Good luck everyone!!

I've had 2 rectocels last year. My syptoms now are feeling like I'm pushing around and object in the rectum and my bowel movement coming out flat. It's as though I've lost my nerve endings to push. So I have to physically bare down to the point I'm afraid I'll have a heart attach. Also, pain during intercourse. Doctor says the U muscle extremely tight and protruding in result causing the pain. Interstim stimulator has been brought up. But reading that is for urinary problems. My question is "will that help?

I have anal itching and pain for approximately the last 6 months. This has been on and off rather that continuous, however, it is extremely painful! Sometimes I am kept awake by the pain and need to take painkillers. When I have bowel movements, I sometimes feel as though there is more to come but I cannot pass it. I have also had some bright red blood when I go to the toilet. Ther also appears to be a little skin tag/lump above my anal opening. It acually looks like it's another hole! i was also suffering from anal problems like fissure and piles both. for some relaxation, take some hot water and sit in this pressuring outside ur anal so that water goes in for atleast 20mins.this is really helpfull aftr that u can use any cream.

After looking on the nhs website i believe i havean anal fissure. i find it difficult to talk to people even close family and friends. i have several issues concerning my body, which is causing other mental health issues eg depression, stress, and possible an eating disorder. i dont feel i could talk to my doctor or even close family and friends as writing this is hard enough. is there any way i can treat any of the problems mentioned without having to see a doctor

i am 16 and have been having stabbing pains in my anal for a few years now, happened about a few times a year (not that often), but this morning i woke up to extreme stabbing pains in my anal and when i tried to walk i experienced more pain. my pains are really extreme and i cannot stand there acting as if its not happening, i end up holding my but, getting on my toes, bending weirdly and making weird noises to how painful it is. i am not going out in public today as i feel i would experience extreme embarrassment if i happened to have one of these pains when people are watching. i am a bit embarrassed to see a doctor about my problem but i really don't want it to be serious and get worse so i want it checked asap.

I feel my stools are just too big to pass sometimes. I feel them scratching out and have to hold onto something! Once it's out there is loads of bright red blood. for the rest of the day it throbs and is generally sore. After reading this I will make an appointment with a female GP. Thanks!

Hi for quite a while now i seem to ear when i pass a large stool. it is never a lot of blood but i am worried that constant bleeding in that area could lead to an infection. i have a same pea sided lump where the bleeding comes from, I thought this was the body trying to protect itself but now I am not so sure. hi i just pulled down my underwear to use the toilet and there was a blood stain on the back part, must've been alot because i chacked my trouser and it had gotten through my underwear slightly and stained my trouser aswel, i didnt feel any pain, or realise i could've been bleeding antil i saw the stain, and the stain looked like it had been there a while, when i checked with a tissue i wasn't bleeding so it had stopped, sometimes when i use the toilet the skin around the anus cracks and little tiny drops of blood come to the surface, is any of this serious? im worried about the blood stain, it hasn't happened before

I have had some stomach problems recently that lead me to being a little constipated and then over the past 3 days, I have been bleeding analy. I fill up the toilet with blood as I am having a bowel movement and then over the past 2 days I have noticed small blood clots as well. One of the blood clots was the size of a tootsie roll and the others are pea size. I also have some bloody mucuous looking stuff when I wipe. I am not on my cycle and no where near it, but I feel like I am with the cramping stomach and all the blood, only it is coming from my anus. Could you give me any advice? I hate going to the doctors.

I have suffered from external piles for a few years and creams have worked fine if i have had a flare up. However for some months now i have had quite a severe pain in the anus after bowel movement which lasts for several hours during the morning and afternoon. It only dies down by the evening and thankfully during the night so i do sleep ok. But once i go to the loo again in the morning, im back to square one again. I thought after reading sites like this i had anal fissures and have tried suppositories and ive even had fibre gel drinks to soften the stools even though i do not have trouble normally, going to the loo. Some days the pain is worse than others, and some days it lasts longer into the day than others. Im scared to go to the doctor, because the thoughts of anything going up my back passage to examine me, makes me feel quite panicky to say the least. But by the same token, im also scared i may NOT have internal piles/fissures and may have cancer or something, as yesterday i felt a lump (which isnt seeming to be IN the anus) but to the side of it. When i feel it,i have to press and i can feel something under the skin inside.it does feel a bit like a needle prick feeling sometimes when i do this. Is this what is putting pressure on my anus wall and causing the fissures? im really worried about it, but keep putting up with it hoping it will go away but ive had this for about 8 or 9 months now with hardly a day out of discomfort, so it aint going anywhere i know...please help

I have been to the docs to have an anal bleeding problem looked at already. I was told that there was nothing to be seen but was given a prescription for it 'just in case it came back'. It has stayed away for a month but I now have another appointment, different doctor, for the same problem, only this time slightly worse. I find blood on the stool, and on the tissue after I wipe. Not a good look, or comfortable, or confidence building. I would say to anyone reading this, you are probably reading it for a reason. Get yourself to the docs - you never know until the experts have a look. This site has put my mind at rest slightly with some useful information. Thank you.

PLease please go to the doctor, tell the receptionist you do not wish to discuss with them they have no right to make you tell them, same goes for a phone call, say I am in an open plan office and wish to discuss it face to face with a doctor. It is your right demand it! Most conditions are very easily treated and most likely to be worms or thrush (very common around the bottom). However, the wrong "self-help" can just make it worse, most creams (especially anything with hydrocortisone or local anaesthestic) and excessive cleaning especially soap or wipes make it worse. Gentle washing 2-4 times a day preferably without soap (use very dilute baby soap and rinse really well) dry with a hairdryer and most of all try not to rub or scratch. An ice pack wrapped in a clean cloth can really help too.

I find the worst part of any doctor's visit is getting past the receptionist. "Whats the problem" is not an easy one to answer, and it's stopped me for years from getting something done. I have no problem discussing with the Doctor, but NOT the receptionist!

I have had excessive sweating for years. I have hade a colonoscopy and found to be clear. However, the sweating and itching is constant. The scratching has sometimes been so intense that I have caused the sking to bleed. Sometimes any anal discharge causes great pain as if the fecal discharge was acid like causing a burning sensation. Sometime my bowel movements are solid and sometimes they are liquid during the course of a day. I have tried powders and washing on a regular basis but with no luck. I feel that the sweating is excessive compared to other parts of my body.

I've had tiny amounts of anal bleeding, only evident after pooing, for years on and off. It was always worse after a heavy week of drinking. It was that infrequent it never really concerned me. All though if I'm honest, I was probably just burying my head in the sand and hoping it would go away. Which it always has, for a while anyway. Things have taken a turn for the more serious today. It seemed like I lost a lot of blood at work today. (in the wc) it's really sacred me. I decided to google anal bleeding and I found this page. I'm now reading about the possibility of bowel cancer, and I'm really sacred. Has putting it off and hoping it will fix it's self all this time given me advanced cancer? Or do I just have bad piles? Either way I'm glad I've recently taken out life insurance, so I know my partner and baby boy will be ok. Off to the docs tomorrow, and not before time...
